My team work with Adults between the ages of 18-65, who have a physical and or sensory disability and or long term health condition to access their community to help combat social isolation and or loneliness. Such as social groups, sport & leisure, volunteering, education. befriending etc. The team receive referrals from Social Workers and Named Workers only. If you would like a referral to the Community Access Support Worker Team contact your named worker/social worker.
